  • Serve as a primary on-site liaison between key provider and/or community partners and Molina
  • Assist Molina members on a walk-in/unscheduled basis
  • Independently problem-solve to connect Molina members to resources
  • Meet expected performance metrics to improve the health and social determinants of health of Molina members
  • Remove access and service barriers for Molina members and onsite provider/community partner
  • Identify ways to improve processes and expand programs to optimally serve Molina members plus overall Medicaid-eligible population
  • Facilitate filing of any expressed member grievance, supporting timely resolution
  • Serves as a community based member advocate and resource, using knowledge of the community and resources available to engage and assist vulnerable members in managing their healthcare needs
  • Minimum 1 year experience working with underserved or special needs populations, with varied health, economic and educational circumstances
