- Once a member has been located, and is engaged in the program, the Outreach and Engagement specialist transitions the member’s case to another team member for ongoing program services
- Utilize available data, such as health service claims, to identify additional outreach opportunities to locate members

- Collaborate with the Department of Children and Families and the Department of Social Services as necessary to support members
- Build, develop and maintain a positive and collaborative relationship with providers and adult consumer and family organizations by establishing a local presence via telephone interactions and by attending meetings
