In the previous role, I was responsible for support with case management services in the form of referrals to outside agencies and management of community-based mental health partnerships within the school.

• Promote access to and effective utilization of mainstream benefits, such as transportation assistance, income and non-income benefits, and SSI/SSDI technical assistance
• Provide services consistent with a Housing First, strengths based, harm reduction and trauma informed approach
• Maintain face to face contact with clients at least monthly, including visual apartment checks, and phone contact with clients as frequently as needed
• Review and update service plans at least annually
• Document all contacts and attempted contacts with a client via a case note within two business days
• Complete annual assessments in HMIS within 30 days before or after the anniversary of the date of engagement

Previously, I was responsible for case management services to refugees and other displaced persons to include coordination of housing, orientation/client education, and other basic needs enrollments within program requirements and time frames utilizing in-house services and referrals to external service providers within the community.

• Knowledge and previous experience working with Microsoft Word, Therap, Office 365, Excel, and web-based housing databases, preferred
• Fluency in Spanish or Haitian Creole
• Conducts orientation with clients and reviews with the family the facility rules, clients’ rights and responsibilities, grievance/complaint process, emergency disaster procedures
• Connect clients to supportive services such as mental health, health, substance use, and employment and education resources with service providers
• Communicate effectively with local community and external agencies
• Help participants negotiate manageable and appropriate lease agreements with landlords and use or develop the skills to be a successful tenant
• Maintain lease and compliance files, all client case files, in an orderly, up-to-date manner
• Provide ongoing, active outreach and creative engagement to tenants
