• Monitoring – The care coordinator will gather sufficient information from all relevant sources in order to determine the effectiveness of the plan of care, including regular prescribed contacts with member and perform updates to the assessment and plan of care as needed
• Able to recognize each patient's unique perspective to care
• Maintain computer skills or systems related to job function
• Certified Medical Assistant (CMA), Certified Nursing Assistant (CNA) or Medical Office Assistant Diploma (MOA)
• Work experience with screening or triage of patients such as in a physician's office, clinic, hospital or long-term care

• Successful completion of CPI training (Parts 1 & 2) within 1 month of hire and annually thereafter
• Extensive experience coordinating care
• Knowledge of HIPAA laws and regulations especially as it refers to the confidentiality of PHI
• Attention to detail, experience with monitoring reports
• Experience in a high functioning, multi-faceted, medical setting preferred
• Familiarity with medical terminology and CPT codes strongly preferred
